MONTHLY REPORT

* FoR GENERATIONS VOLUNTEERS ONLY *

Click here for the April 2023 Monthly Form
We ask that all GENERATIONS VOLUNTEERS take a moment each month to complete a report on your volunteer hours and activities. The link above will take you to LimeSurvey, where we have set up a brief questionnaire for you to complete - it will take less than 5 minutes. We ask that you please add a monthly reminder to your personal calendar to carry out this task. Please note, this step is part of your volunteer mandate, as outlined in the Volunteer Agreement you signed and submitted to the Program Coordinator.

The information you report each month is critical to the YD operations. Not only does it help us better understand our volunteer and member needs, but is also where you can report any difficulties/situations that require the attention of the Program Coordinator. We check the reports from volunteers regularly. Additionally, the data collected from our volunteers is required by many of our funders (as demonstrated in our Annual Report) - the information you provide helps us to measure our impact in the community and to maintain important relationships with our major funders.

Thank you!

We acknowledge that The Yellow Door is situated on unceded Indigenous land, as it was taken without treaty or agreement. We recognize the Kanien’keha:ka Nation as the custodians of Tiohtia:ke (Montreal), a traditional meeting place with other Iroquoians and Algonquians, including the Algonquin-Anishinaabe, Atikamekw and the Huron-Wendat. We stand in solidarity and in action in supporting the journey towards reconciliation.
